Job summary

CASES, a New York City agency focused on behavioral health and involvement in the criminal justice system, seeks a Case Manager in the Bronx to deliver comprehensive care management, coordination, and health promotion. You will work with a multidisciplinary team to support clients in goal planning and resource linkage.

You will assist with insurance enrollment, housing, and entitlements, and collaborate to identify services and treatment objectives.

Qualifications

• Bachelor’s degree in human or social services or related field.
• At least 2 years of work experience providing peer support to individuals with serious mental illness and/or substance use disorder and criminal justice involvement.
• Individuals with lived experience with mental health, substance use or criminal legal experience are encouraged to apply.
• Proficiency with computer and electronic health records.

Responsibilities

• Assist with developing person-centered goal planning.
• Screen and assess social drivers of health and assist with housing instability, food, transportation needs and other concerns impacting wellness.
• Collaborate with participants and care management team to identify services and treatment objectives.
• Provide care coordination and health promotion to program participants.
• Assist eligible clients to enroll in health insurance/Medicaid and identify entitlements.
• Provide participant and family support and referrals to community and social supports.
