Job summary

Twelve Oaks in Navarre, FL is seeking a compassionate Case Manager to coordinate patient care, support treatment planning, and facilitate discharge and aftercare services. This full-time role offers collaborative work in a behavioral health setting with a focus on quality outcomes.

Ideal candidates will have a Bachelor’s degree in Social Work or related field and at least one year of behavioral health experience, with CPR/de-escalation certification preferred.

Qualifications

• Bachelor’s degree in Social Work, Counseling or related human service field required.
• One or more years’ experience working in a behavioral health setting preferred.
• CPR and de-escalation/restraint certification required (training available upon hire).
• First aid may be required based on state or facility.

Responsibilities

• Provide case management and care coordination services for clients/patients/residents in the facility.
• Coordinate continuum of care activities for assigned clients and ensure optimum utilization of resources and compliance with requirements.
• Conduct psychosocial assessment and individual, family and group sessions.
• Coordinate treatment and discharge activities as needed.
• Develop, implement, manage and evaluate client/patient care plans.
• Collect, correlate and provide clinical data to the treatment team.
• Communicate medical criteria and information between insurance providers and treatment team as warranted.
• Coordinate relevant issues or activities for the treatment team in a timely manner.
