Job summary

Safe Harbor Recovery Center in Virginia is seeking a Case Manager to support multiple units by coordinating care and discharge planning within the treatment team. You will maintain comprehensive clinical records and collaborate with families, providers, and referral sources to ensure the ISP is followed.

The ideal candidate has at least a High School diploma, CPR and de-escalation training, and prior case management or behavioral health experience.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Social Work, Counseling or related field preferred.
• One or more years of related behavioral health experience preferred.
• Experience in case management preferred.

Responsibilities

• Develops and coordinates individualized service plans across units.
• Maintains clinical records and documents progress toward recovery.
• Collaborates with treatment team, families, and providers to implement ISP.
• Ensures discharge planning begins at admission and coordinates referrals.
• Coordinates AHCCCS/SSI/SSD eligibility processes where applicable.
