Job summary

Qualifications

• Minimum of two years' direct service experience providing case management to older adults and adults with disabilities.
• Minimum of two years' experience applying assessment and evaluation techniques, including communication, records, and confidentiality.
• Minimum of two years' experience with interviewing, counseling, crisis support, and developing care plans.
• Ability to relate to people from diverse backgrounds.

Responsibilities

• Conduct client interviews to assess needs, risks, strengths, and eligibility for programs or services.
• Develop, implement, and monitor individualized service plans.
• Coordinate services and navigate community resources for clients.
• Advocate for clients to access services and monitor progress.
• Maintain compliant case documentation and prepare required reports.
