Qualifications

• Associate degree in nursing or higher and Alabama nursing license.
• Five years of clinical experience in acute care settings required.
• Home Health, Hospice or Case Management discharge planning experience preferred.
• Strong communication and organizational abilities essential.

Responsibilities

• Coordinate with physicians, nurses and health team to expedite care.
• Support physician provision of patient care with accountability for caseload.
• Adhere to clinical guidelines, tools and protocols based on evidence quality.
• Promote resource utilization and monitor outcomes with multidisciplinary team.
• Apply process improvement methods to evaluate outcomes of care.
• Coordinate discharge planning across continuum of care.
• Assist patients in gaining access to essential services.
• Demonstrate teamwork and deliver exceptional patient experiences.
• Assess safe discharge needs and facilitate transition to next level of care.
• Uphold patient safety and quality standards.
