The Case Manager supports residents in achieving independence and stable housing by providing case management, service planning, and coordination of care. This role involves developing and implementing individualized service plans, maintaining documentation (ILPs, psychosocials, and progress notes), and collaborating with internal staff and external providers to ensure residents receive appropriate housing, healthcare, and support services.
