ICL is seeking a Case Manager for Supported Housing. The role supports individuals in independent apartments in the community, coordinating access to medical, psychosocial, financial, vocational, and other essential services, with crisis intervention as needed.
Under supervision of the Program Director, you will travel to client residences, conduct assessments, maintain case records, provide counseling, and connect clients to community supports, educational opportunities, and disability
Education: Bachelor's degree in Social Work, Human Services, or related field
Experience: Bachelor's degree plus two (2) years of experience providing direct services to individuals with mental disabilities or related human services experience; OR Bachelor's degree with a one-year practicum (e.g., BSW) and one (1) year of relevant experience

Work Eligibility: Mustbe authorized towork in the U.S.
Work Location Requirement: Onsite/community-based (regular travel to client residencesrequired)

Fixed Rate: $26.52 per hour
