ICL in New York City seeks a Case Manager for Supported Housing to assist individuals with mental health needs in securing stable housing and coordinating medical, psychosocial, financial, and vocational services. You will deliver crisis intervention and support independence in the community.
Under Program Director supervision, you will travel to client residences, document plans, advocate for clients, and participate in trainings to ensure safe, quality care and recovery-oriented supports.
Education: Bachelor's degree plus two years’ experience providing direct services to individuals with mental disabilities or other relevant human service experience, or bachelor's degree that required a one-year practicum working directly with individuals with mental disabilities (e.g., BSW) and one year experience, or associate's degree plus 3 years above experience.
