Qualifications

  • Bachelor’s degree in human services or related field preferred, with a minimum of two years of experience.
  • Experience with diverse and/or low-income populations, including homelessness or unstable housing and/or mental illness.
  • MNsure Certified Navigator/Assister experience preferred.
  • Experience as a Coordinated Entry assessor, Ramsey County, preferred.
  • Experience or training in trauma-informed care, harm reduction, motivational interviewing, crisis intervention, and de-escalation.

  • Collaborates with the interdisciplinary team to identify barriers, coordinate services, and support discharge planning.
  • Maintains timely, accurate documentation and records.
  • Tracks client progress and follows up on referrals and service connections.

JOB SUMMARY

The Medical Respite Case Manager II delivers trauma-informed, person-centered case management services to individuals experiencing homelessness or housing instability who are recovering from acute medical conditions. Serving clients with complex medical, behavioral health, substance use, housing, and social service needs, this role works collaboratively within an interdisciplinary team to assess needs, develop individualized service and discharge plans, and coordinate access to community resources and support services.

The Case Manager serves as a primary advocate for assigned clients, conducting comprehensive intake assessments, identifying strengths and barriers, and connecting individuals to benefits, housing resources, healthcare services, and community programs. This role monitors client progress, adjusts service plans as needed, maintains accurate and timely documentation, and supports successful transitions from Medical Respite to stable housing and community-based services.

Additionally, the Case Manager partners with healthcare providers, community organizations, and internal programs to ensure continuity of care, promote client self-sufficiency, provide education on available resources, and advocate for equitable access to services that support long-term health and stability.

WAGE RANGE:

$25.00–$28.00/hour

ESSENTIAL FUNCTIONS
  • Completes comprehensive client assessments and develops, monitors, and adjusts individualized service and discharge plans based on client needs, goals, strengths, and barriers.
  • Provides trauma-informed, person-centered case management to individuals experiencing homelessness or housing instability who are recovering from acute medical conditions and may have complex medical, behavioral health, substance use, housing, and social service needs.
  • Assists clients with navigating public benefits, health insurance, housing and Coordinated Entry systems, identification/documentation needs, and other resources necessary to support discharge planning and long-term stability.
  • Coordinates referrals, linkages, crisis response, and follow-up with hospitals, healthcare providers, housing providers, behavioral health services, and other community partners to support continuity of care and successful transition from Medical Respite.
  • Collaborates closely with the Medical Respite interdisciplinary team, including nursing, behavioral health, case management, and other program staff, to identify barriers, coordinate services, and support client-centered goals and discharge planning.
  • Supports client autonomy and informed choice while using engagement, problem-solving, and de-escalation strategies to address barriers, crises, and changing client needs.
  • Maintains timely, accurate, and complete case documentation and records in accordance with agency, program, funder, and applicable regulatory requirements.
  • Tracks client progress, follows up on referrals and service connections, communicates significant changes to appropriate team members, and modifies service plans as necessary.
  • Participates in interdisciplinary case consultation, team meetings, and coordination with community partners to promote effective service delivery and continuity of care.

QUALIFICATIONS/TECHNICAL EXPERTISE
  • Bachelor’s degree in human services or related field preferred, with a minimum of two (2) years of experience working with a specific client group.
    • In lieu of degree, three (3) years of directly related experience required.
  • Experience working with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness.
  • MNsure Certified Navigator/Assister experience preferred.
  • Experience as a Coordinated Entry (CE) assessor, particularly within Ramsey County, preferred.
  • Experience or training in trauma-informed care, harm reduction, motivational interviewing, crisis intervention, and de-escalation.
