Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.Position Summary:The Case Manager 2 is responsible for providing home- and community-based case management services to homeless young adults located throughout the state.
