Job summary

The Connection, Inc. is seeking a Case Manager 1 at Eddy Shelter in Middletown. The hourly rate is $22.00 and the role is full time (40 hours) on the 1st shift (Monday–Friday, 10:00 am–6:00 pm).

This position provides staff support and direct client care, delivering comprehensive case management services with a focus on housing stability. You will conduct intake assessments, develop individualized care plans, and coordinate with treatment providers and community resources to support clients

Qualifications

• Some Post-secondary education.
• 2+ years related experience in the direct service and case management field.
• Valid Connecticut Driver’s License.

Responsibilities include delivering comprehensive case management services to clients including intake assessments, development and implementation of individual care plans, and coordinating with treatment providers and community resources.
