Job summary

Epic Healthcare in Santa Cruz, CA is seeking a dedicated Case Manager to support Veterans with housing and services, and to coordinate with VA and community resources.

You will develop service plans, provide residential support, and advocate for residents’ needs while maintaining thorough documentation and progress notes. A military or veteran background and strong communication are essential.

Qualifications

• High School Diploma/GED required.
• Bachelor’s degree in Social Work or a related field preferred.
• Two years of related experience preferred.
• Military or Veteran experience strongly preferred.
• Knowledge of Veteran services and community resources is a plus.
• Valid First-Aid certification required.
• Must pass a criminal background check.
• Must be at least 18 years old.

Responsibilities

• Provide case management and residential support to Veterans

• Connect Veterans with VA, medical, social, educational, and community resources
• Assist with housing searches, appointments, outreach, and discharge planning
• Facilitate structured groups and living skills activities
• Monitor residents’ progress and well-being

• Collaborate with VA personnel, families, providers, and the care team
• Assist residents with daily living activities when needed
