Job summary

  • 2+ years in community resource acquisition and working with individuals with mental health or co-occurring disorders.

Responsibilities

  • Conduct comprehensive assessments to determine mental health, social, medical, housing, and vocational needs.
  • Develop and complete Person-Centered Treatment Plans (PCTP) and reviews with clients and families.
  • Provide services outlined in the PCTP including case management and individual skills training.
  • Monitor client progress and adjust service plans accordingly.
  • Enter progress notes, maintain scheduling in EHR, and complete billing documentation on time.
  • Advocate for clients’ access to housing, benefits, employment, and healthcare.
  • Coordinate care and communicate with internal and external team members.

Job description

The Case Manager provides coordinated, client-centered services to individuals and or their families experiencing both mental health challenges and substance misuse. This role supports clients in achieving stability, independence, and improved quality of life by assessing needs, developing service plans, coordinating care, and connecting clients and or their families to community resources.

Monday-Friday 8A-5P

essential functions

The essential functions may include, but are not limited to the following:

  • Conduct comprehensive assessments to determine mental health, social, medical, housing, and vocational needs.
  • Develop and complete Person-Centered Treatment Plans (PCTP) and treatment plan reviews with clients and their families within the timeline identified by policy.
  • Provide services to clients and their families as outlined in the PCTP including case management and individual skills training.
  • Monitor client progress and adjust service plans accordingly.
  • Enter progress notes, complete State Registration and assessments, maintain schedule in EHR, and complete billing documentation within the timeline identified by policy.
  • Participate in team meetings, communicate and cooperate with both internal and external team members regarding tasks, and contribute actively to the team’s efforts to ensure the clients and their family’s needs are met.
  • Meet with clients and or their families based upon the level of need of the state assessment and determined by the team.
  • Provide crisis intervention and risk assessment as needed.
  • Advocate for clients’ access to services including housing, benefits, employment, and healthcare.
  • Maintain knowledge of Medicaid Rehabilitation Option (MRO) guidelines regarding provision of services, documentation requirements, qualifying diagnoses, and level of need issues.
  • Follow expectations set forth by the Family Health Center in regard to training, productivity, policy, and documentation.
work-related experience
  • Prefer [2] years of experience in community resource acquisition and working with individuals with mental health or co-occurring disorders.
  • Experience working with diverse populations, including individuals with complex medical or behavioral health needs.
