Empowered Milestones, LLC in San Jose seeks a Case Manager to support intellectually disabled clients in the Supportive Living Services team. The role emphasizes partnership development, service quality, and strong internal collaboration to ensure clients’ rights are protected.

Travel within the county is required, and reliable transportation is essential. This non-remote position offers benefits and opportunities to contribute to community living services in the San Jose area.

Qualifications

• Experience: Minimum of 1 year of management experience, preferably in the assisted living community.
• Requirements: Knowledge in Microsoft Office Suite; Driving to other offices or counties may be required and must be able to respond to emergencies.
• This is NOT a remote position, and requires reliable transportation and ability to travel within the county area.

• Form strong internal relationships and advocate for client rights.

We are adding another manager to our San Jose Supportive Living Services Team.

The Case Manager supports the Supportive Living Services for intellectually disabled clients living in San Jose area.

Schedule:

Monday - Friday
