Rehabilitation Hospital of Northwest Ohio in Toledo, part of Ernest Health, is seeking a Case Manager to join our inpatient rehab team. You will work with physicians and a multidisciplinary care team to develop individualized plans and monitor patient progress toward outcomes.

The role emphasizes active discharge planning, coordination with families and community resources, and alignment with our guiding principles.

Qualifications

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.

Responsibilities

• Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
• Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records.
• Identifies and resolves barriers that hinder effective patient care.
• Actively involved in discharge planning process.
• Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Qualifications
