Job summary

St. Patrick Center, part of Catholic Charities of the Archdiocese of St. Louis, seeks a Behavioral Health Intake Counselor to complete intake, assessment and screenings for new behavioral health clients.

The counselor will provide counseling, crisis intervention and case management as needed, all from a trauma-informed perspective and in collaboration with the Intensive Case Management team. This role interfaces with St.

Qualifications

  • Degree in counseling, social work or psychology preferred.
  • Experience providing case management and counseling.
  • Experience providing crisis intervention and trauma informed services.
  • Experience with wrap-around services preferred.

Responsibilities

  • Intake and assessment of all new clients entering the day treatment program.
  • Provide counseling, crisis intervention and case management as needed.
  • Accurately enter all client activities and provided services in CaseWorthy and HMIS client tracking systems within one business day of contact.
  • Review and clean up entry/exit data in HMIS monthly.
  • Attend quarterly HMIS and PATH User Meetings.
  • Run reports in HMIS as assigned.
  • Maintain and update client records as needed.
  • Maintain accurate filing system.
  • Participate in weekly ICM team meetings and clinical supervision.
  • Collaborate with the ICM team.
  • Carry small caseload of MH/SUD clients in ICM.
  • Attend community Weekly Housing and Match Meeting.
  • Act as point person for all Shamrock housing referrals made during WHAMM.
  • Collaborate with Program Assistant daily, assist with administrative tasks as needed.
  • Keep Memorial Service list current.
  • Plan and execute Memorial Service annually.
  • Facilitate group education meetings as needed.
  • Refer clients to appropriate entitlement agencies and community services
  • Network with other related agencies
  • Respect client confidentiality, comply with HIPAA regulations
  • Responsible for agency policy regarding work hours, building security and all written personnel standards
  • Ensure the care model by interacting with all other St. Patrick Center programs
  • Research additional benefits available to enhance services
  • Other duties as assigned

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.

Job Summary

Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is looking for a Behavioral Health Intake Counselor who will complete intake, assessment and individual mental health and substance use disorder screenings of all new behavioral health clients. The counselor will provide counseling, crisis intervention and case management services as needed. All duties must be performed through a trauma informed care perspective and will work closely with the Intensive Case Management (ICM) team, as well as other St. Patrick Center programs and appropriate community agencies.

Knowledge and Experience Requirements
  • Degree in related area of counseling, social work or psychology preferred
  • Experience providing effective case management and counseling
  • Experience providing crisis intervention and trauma informed services
  • Experience with wrap-around services preferred
