Care Manager (also known as a Waiver Service Coordinator) is responsible for conducting in-home assessments, identifying support systems, and coordinating long-term services through the MyCare Ohio Waiver Program. This role ensures members receive appropriate care in home- and community-based settings while maintaining compliance with Medicaid and program regulations.
This position is primarily office-based in a climate-controlled environment with regular in-home visits. Requires extended computer use and occasional lifting to 25 lbs.
Hiring is contingent upon successful completion of a drug screen, background check, and reference check. AOoA is a drug-free workplace. Employment is contingent upon funding availability.
