Buckeye Hills Regional Council is currently seeking a full-timeCare Management Coordinator for Washington, Athens, or Hocking Counties.The Care Management Coordinator utilizes the skills and knowledge from their profession as an RN or LSW with minimal direction from their supervisor to implement, evaluate, and coordinate waiver services that will enable eligible individuals to remain in the community safely, avoiding institutionalization.
The Care Management Coordinator independently develops and maintains comprehensive, person-centered care plans based on individual strengths/weaknesses, assessed needs, and formal and informal supports. They ensure that care plans are modified to meet individual needs and changes as they occur. They maintain regular contact with individuals and caregivers either by telephone or in-person visit as per the Ohio Department of Aging standard to monitor individual satisfaction, appropriateness of care, and ongoing quality of service delivery.
Education/Experience Required:Must be a Registered Nurse or Licensed Social Worker with a valid and current license in the State of Ohio and one (1) year experience.
Starting Salary Range: $37,189 – $46,486.
Fingerprinting, background check, pre-employment drug testing, and a valid driver’s license are required for hire.
