Job summary

The Gathering Inn in Sacramento seeks a Case Manager to guide guests toward stability and self-sufficiency. You will conduct comprehensive assessments, develop individualized care plans, and coordinate access to housing, health, and supportive services.

This full-time, non-exempt role requires a calm, empathetic presence and strong documentation skills using HMIS. You’ll work Sunday through Thursday, 8:00 AM to 4:30 PM, and earn $28.00 per hour, with Live Scan clearance expected.

Qualifications

• Bachelor's degree with one year of experience or an equivalent blend of education and practical experience.
• Equivalent may include high school diploma with five years of relevant experience.
• Associate's degree with three years of experience will also be considered.
• Computer skills for documentation and record-keeping.
• Ability to pass a Live Scan background check.

Responsibilities

• Conduct comprehensive assessments of guests’ needs, strengths, and goals and develop individualized care plans.
• Maintain accurate records of guest interactions, services, and outcomes using HMIS.
• Coordinate access to services and resources, ensuring eligibility and follow-through.
• Collaborate with community partners to advocate, schedule appointments, arrange transportation, and refer.
• Lead assigned classes or programs for guests.
• Uphold guest confidentiality and data privacy.
• Address behavioral concerns calmly and respectfully, maintaining safety.
• Participate in staff meetings and trainings, including De-Escalation, Motivational Interviewing, CPR, and First Aid.

Originally a winter-only shelter serving 40 individuals nightly, we have evolved into a multifaceted organization with multiple locations, serving nearly 500 individuals and families daily. What You’ll Do

As a Case Manager, you play a vital role in helping guests move toward stability and self-sufficiency. You will:

• Conduct comprehensive assessments of guests’ needs, strengths, and goals through initial intake, and develop individualized care plans in collaboration with guests
• Maintain accurate and up-to-date records of guest interactions, services provided, and outcomes, utilizing the Homeless Management Information System (HMIS)
• Coordinate access to service providers and resources, ensuring eligibility, follow-through, and seamless delivery of support
• Work collaboratively with community partners to advocate for guests, schedule appointments, arrange transportation, and make referrals
• Facilitate and lead assigned classes or programs for guests
• Uphold guest confidentiality and data privacy standards
• Address behavioral concerns calmly and respectfully, maintaining safety and harmony
• Participate in staff meetings, in-service training, and other professional development opportunities, including De-Escalation, Motivational Interviewing, CPR, and First Aid

Minimum Qualifications

• A bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience is necessary.
