Job Overview

Clinical Case Manager – Per-Diem. BHC Alhambra Hospital is located in Rosemead, California. The 115-bed acute psychiatric facility provides inpatient, residential, partial hospitalization, and intensive outpatient services for children, adolescents, and adults.

Key Responsibilities
  1. Participate in implementation of the treatment plan and conduct ongoing reviews to ensure effective implementation of all interventions.
  2. Maintain ongoing direct contact with the attending practitioner, treatment coordinator, and various members of the team.
  3. Assist in ensuring that the patient attends the appropriate level of care or program as indicated on the treatment plan or when changes in the patient's status occur.
  4. Interface with staff of the appropriate level of care to facilitate smooth transitions at the time of transfer.
  5. Maintain necessary documentation of all services and quality of care.
  6. Assure proper tracking of all reviews.
Requirements
  • Education: Masters degree from an accredited college or university in social work, mental health, psychology, or a related field.
  • Experience: Minimum four (4) years of direct clinical experience in a psychiatric or mental health setting, with experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.
  • Skills: Strong written, verbal, and interpersonal communication skills.
