Fort Lauderdale Behavioral Health Center, a subsidiary of UHS, seeks a Case Manager (BSW) to support discharge planning and aftercare coordination for adults and adolescents in a private 182-bed hospital. You will work closely with the treatment team to ensure seamless placement and community referrals.
The role requires a Bachelor's in psychology or related field, CPI or equivalent, and CPR, with a minimum of two years in psychiatric or mental health case management.

CASE MANAGER (BSW)
The Case Manager functions under the direct supervision of the Director of Social Services. He or she assumes responsibility for working with the treatment team and the patient in order to assist in basic discharge planning needs, coordination of aftercare and placement of the patient. This includes residential needs, aftercare needs, advocacy needs, entitlement needs, and building relationships with aftercare providers. Other duties as assigned by supervisor.

Experience: A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in case management patient assessment, patient aftercare coordination, and communication with external review organizations. Experience in working with community resources, residential/outpatient programs and outside referrals.
Knowledge and Skills: Extensive knowledge of community resources and FLBHC's referral sources in order to make appropriate referrals.
