Job summary

Charlotte Behavioral Health Care in Florida seeks a qualified professional to assess needs, develop service plans, and link adults and children to community resources. You will ensure services align with regulations and coordinate transitions, working closely with caregivers to address complex needs.

This role manages multiple high-priority crises, maintains EMR documentation, and may transport clients as required. A 40-case load is expected, with children counted as two.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, or related field.
• Preference of 1 year full-time or equivalent experience working with individuals with serious mental illness.

Responsibilities

• Assess needs and develops service plans based on identified needs.
• Links individuals to appropriate community service agencies.
• Provides services in accordance with 65E-15 FS and Medicaid regulations.
• Coordinates effective transition/discharge planning.
• Coordinates with caregivers in reference to individual needs.
• Coordinates multiple complex priority crises.
• Manages patient placements and placement options.
• Manages a maximum caseload of 40 individuals; children count as two.
• Documents all interactions in the EMR.
• Transports individuals served when needed.
• Provides services in individual/group homes and other community locations.
