Job summary

Danville-Pittsylvania Community Services seeks a dedicated professional to deliver intensive, short-term assessment, linkage, counseling and advocacy for adults, children, and families facing behavioral health crises. The role includes developing service plans and coordinating with care providers to promote recovery and community functioning.

The position requires compliance with licensure and payer documentation, plus a valid driver’s license and safe driving record.

Qualifications

• Requires a Bachelor's degree in Psychology, Social Work, Counseling, Sociology, Criminal Justice, or other approved Human Services degree.
• Must have a valid driver's license and a safe driving record.

Responsibilities

• Provide intensive, short-term assessment, linkage, counseling and advocacy to adults, children and families in crisis.
• Assist in developing and implementing individual services plans and coordinate with other providers.
• Complete required documentation for licensure, Medicaid and other payers according to regulations.
• Support caregivers and help individuals live and function in the community.

Responsibilities

Provides intensive, short term assessment, linkage, counseling, and advocacy services to adults, children, and families who are experiencing a behavioral health crisis, in vivo at a level of intensity necessary to promote individual empowerment and recovery and reduce the need for inpatient treatment; assists in the development and implementation of individual services plans; provides support services to care givers; completes required documentation as prescribed by licensure, Medicaid, managed care groups or other third party payers with regards to clinical services and medical records keeping; and complies with all regulations applicable to program services; works as part of the team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.

Requirements

• M1: Bachelors degree in Psychology, Social Work, Counseling, Sociology, Criminal Justice, or other approved Human Services degree
• M2: Valid driver's license and safe driving record
