Qualifications

  • Bachelor’s degree and one year of experience preferred OR equal combination of education and experience.
  • Intermediate computer skills and ability to establish a HIPAA compliant home office.
  • Must pass drug screening and state and federal background checks.
  • Previous experience in behavioral health, human services, case management, or residential care settings is preferred.

Responsibilities

  • Act as part of a multidisciplinary team designed to provide services from a holistic and values‑based perspective.
  • Assist in the development of service plans and reviews with the program participant, medical team staff and other stakeholders.
  • Develop and maintain relationships with community agencies to assist participants in connecting with resources.
  • Facilitate team process for the initial and annual development of the Individual Service Plan (ISP).
  • Ongoing monitoring of ISP and documenting results and intervening as necessary.
  • Report and review Critical Incident Reports (CIR) per protocol.
  • Participate in Human Rights Committee and Behavioral Intervention Committee reviews.
  • Focus on Personal Outcome measures and Charting the Life Course framework.

What You'll Do

Act as part of a multidisciplinary team designed to provide services from a holistic and Values

based perspective.

Assist in the development of service plans and reviews in collaboration with the program

participant, medical team staff and other stakeholders as determined in collaboration with

Develop and maintain relationships with community agencies in order to assist program

participants in connecting with all available resources.

Facilitate team process for the initial and annual development of the Individual Service Plan (ISP)

Ongoing monitoring of ISP and documenting results and intervene as necessary

Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee

(BIC) reviews as appropriate

Centered Thinking/Planning

Evaluate the needs of those on caseload in order to remain successful in the community

Link those on caseload to programs and supports that are needed and requested to meet

their needs

Complete required documentation of services, including input of data, reports, and plans into

Therap.
