Job summary

The Children’s Home of Reading in Reading, PA is seeking a Foster Care Case Manager to support youth and foster families within a mission-driven team. This full-time role emphasizes compassionate service, professional conduct, and adherence to child welfare standards.

Responsibilities include intake, service planning, ongoing coordination, crisis intervention, and maintaining confidential records. Training is provided; prior experience is valued but not required for all positions.

Qualifications

  • Bachelor’s degree in social work, psychology, or related field.
  • One year verifed experience in case management or counseling with children and families (drug and alcohol experience appreciated).
  • At least three written references demonstrating past educational or employment experience.
  • Must be at least 21 years of age.
  • Valid PA driver’s license with good driving record; maintain it throughout employment.
  • PA Act 33/34 clearances and DPW FBI clearance.
  • Pre-employment physical showing no communicable diseases.
  • Must meet E-Verify requirements for employment eligibility.

Responsibilities

  • Completes intake for new clients and writes individualized service plans.
  • Orients youth and families to SafeGuards programming.
  • Maintains a caseload of 12–15 children and their foster families.
  • Has bi-weekly face-to-face contact with each child and family.
  • Maintains weekly telephone updates with the referring agency.
  • Transports clients to home visits and other locations as needed.
  • Plans and provides crisis intervention as required.
  • Keeps client files per policy and confidentiality/HIPAA standards.
  • May assist in implementing a ½ hour Social Skills group.

What You’ll Do

As a Foster Care Case Manager, you will:

  • Ensures one’s own training requirements are completed and up to date and reported to the Program Manager.
  • Completes the intake process for new clients, obtaining the appropriate documentation for placement. facilitates and writes individualized service plans and ensures on-going coordination of the service plan.
  • Orients new youth and families to SafeGuards programming.
  • Maintains a caseload of 12 to 15 children and their foster families.
  • Has face-to-face contact with each referred child and his/her foster family at least on a bi-weekly basis (may require extensive travel based on the individual needs of the child and family).
  • Has telephone contact with each child’s referring agency each week to provide updates and youth activities within the program.
  • Transports assigned clients to and from home visits and other locations as necessary.
  • Plans, arranges, and/or provides crisis intervention services as needed.
  • Maintains client files according to standards and as required by policy and procedures.
  • Maintains confidentiality and HIPAA standards for all assigned clients and families.
  • The Case Manager may need to assist in the implementation of a ½ hour Social Skills group.

Qualifications

Required:

  • Bachelor’s degree in social work, psychology, or related field
  • One (1) year verified experience in case management or counseling with children and families with one (1) year experience. (Drug and alcohol programs - one year drug and alcohol experience required.)
  • At least three (3) written references that demonstrate past educational or employment experience.
  • Must be at least 21 years of age.
  • Valid PA driver’s license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage. Must maintain license and good driving record throughout employment at CHOR.
  • PA Act 33/34 clearances and DPW FBI clearance.
  • Pre-employment physical from a licensed physician (after conditional job offer). Verifying person has no communicable diseases and no physical conditions that might endanger the health and safety of children or prevent the person from performing essential job responsibilities.
  • Must meet all requirements of Title 8, United States Code, Section 1324-A of the United States Department of Justice/immigration and Naturalization services Employment Eligibility Verification.
  • Availability and willingness to work a flexible schedule.
