Job summary

The Children's Home of Reading is seeking a compassionate Case Manager for the ORR program to support UACs across Bethlehem and Doylestown, PA. This full-time role emphasizes independent judgment, collaboration with sponsors, and timely documentation in the UAC Portal.

Responsibilities include release planning, coordination with foster families and clinicians, completing ORR trainings, and maintaining strong communications with children and families. Paid training provided.

Qualifications

  • Bachelor's degree in behavioral sciences, human services, or social services fields is required.
  • Must be at least 21 years of age and able to pass ORR background investigations.
  • Proof of immunity to vaccine-preventable diseases and compliance with health screening requirements including TB is required.
  • Must complete ORR pre-service training and annual continuing education; sign CHOR Code of Conduct and reporter agreement.

What You’ll Do

As a Case Manager, you will:

  • Maintains a caseload consistent with ORR case management ratio requirements.
  • Provides weekly individual meetings with assigned UACs.
  • Coordinates communication with sponsors regarding release planning.
  • Manages release planning activities, including home study coordination and travel arrangements.
  • Documents all case activity in the UAC Portal in a timely manner and accurate manner.
  • Coordinates with foster families, clinicians, and Lead Case Manager on service needs.
  • Provides child-facing services on-site within the assigned service area.
  • Participates in case review meetings.
  • Completes required training on child welfare, release planning, and mandatory reporting.
  • Exercise independent judgement consistent with program policy, ORR requirements, and applicable law.
  • Communicate effectively, verbally and in writing, with children, families, and staff.
  • Maintains regular, reliable attendance and respond promptly to program needs, including outside standard business hours when required by the role.

Qualifications

Required:

  • Bachelor's degree in behavioral sciences, human services, or social services fields.
  • Must be at least 21 years of age.
  • Must exhibit integrity and good moral character appropriate for working with children in a residential or foster care setting.
  • Must successfully complete ORR-required background investigations.
  • Must provide proof of immunity to vaccine-preventable diseases transmitted by the respiratory route (Varicella, MMR, Tdap, annual Influenza) and comply with all state licensing health screening requirements including TB screening.
  • Must complete all ORR-required pre-service training, as required or scheduled by ORR.
  • Must complete all ORR-required continuing education on an annual basis, as required or scheduled by ORR.
  • Must sign and comply with CHOR's Code of Conduct consistent with ORR policy, confidentiality acknowledgment, and mandatory reporter agreement.
  • Must disclose any prior misconduct, including conduct occurring off duty, on the employment application and in interviews. Material omissions are grounds for termination.

Preferred:

  • Child welfare and/or case management experience strongly encouraged.
  • Bilingual candidates preferred given the program's served population.
Schedule
  • Full-Time / Exempt
  • This position is based across two work locations, Bethlehem and Doylestown. The employee will be required to report to and work from both offices as necessary and assigned.
Physical Requirements
  • Ability to sit at meeting table or desk, stand, walk, bend, lift, reach, stoop, twist, climb steps, drive a vehicle and assist individuals as needed.
  • Ability to perform essential job functions with or without reasonable accommodation
