All In Solutions in Eatontown, NJ is seeking a dedicated Case Manager to join our clinical team. The Case Manager will serve as a vital link between clients, treatment providers, and community resources to support recovery.

This full-time role runs 8am-4:30pm, Monday through Friday, and focuses on assessments, service planning, and advocacy. You will coordinate with housing, employment, legal aid, and medical care while maintaining thorough documentation.

Qualifications

• Bachelor's degree in psychology, counseling, social work, or a related field, preferred; relevant certification or licensure preferred.
• Experience with substance use disorders and co-occurring mental health issues.
• Proficiency in documentation and electronic health records.
• At least 1 year of experience in case management or a behavioral health setting.

Responsibilities

• Conduct initial and ongoing assessments of clients’ psychosocial, environmental, and financial needs.
• Develop and implement individualized service plans with clinical staff and the client.
• Coordinate services with internal departments and external agencies (housing, employment, legal aid, medical care).
• Advocate for clients to ensure access to appropriate treatment and support.
• Maintain detailed documentation in compliance with standards.
• Participate in case conferences and training sessions; provide crisis intervention when needed.
