Description

Seeking high energy and responsible individual to work collaboratively as a Case Manager with women who have a Mental Illness. Candidate will work with clients to support wellness, recovery, independent living skills as well as provide a full range of case management, advocacy and liaison services in our Residential Program in Boro Park, Brooklyn. Responsibilities include, but are not limited to, regular apartment visits and inspection, medication and medical monitoring in conjunction with the care coordinators in the Health Homes, linkage to community resources, the facilitation of client empowerment and choice, and contact with therapists, psychiatrists, peer specialists, and other natural supports. Position requires a Bachelor’s degree in the Human Services field, (Masters preferred) and is full time (some evening hours required).
