Job summary

The Bridge in New York seeks a Residential Case Manager to support formerly homeless adults in Manhattan, Brooklyn, and the Bronx. You will hold individual meetings, co-facilitate groups, and maintain timely documentation under the supervision of the Program Director.

You will provide housing assistance, referrals, benefits maintenance, assessments, and coordination with external providers, using Motivational Interviewing to support housing stability.

Qualifications

  • Experience working with mental health and crisis intervention.
  • Ability to manage trauma-triggered behavior and maintain organization.
  • Willingness to perform CPR/First Aid/AED/CPI/Naloxone and work with diverse populations.

Responsibilities

  • Deliver comprehensive case management including housing assistance, referrals, benefits maintenance, assessments, and coordination with external providers.
  • Collaborate with the clinical High-Risk Coordinator to provide case management and interventions.
  • Create Support Plans in the electronic client records system and document timely case notes.

Job Title:

Residential Case Manager – Manhattan, Brooklyn, and The Bronx

Educational Requirements:

Bachelor's Degree or high school diploma/GED with relevant experience.

Qualifications and Experience:

Experience working with mental health, crisis intervention, and managing trauma-triggered behavior. Excellent time management skills; stress-resistant and flexible; team player; open to feedback; creative; excellent organizational skills; willing to perform CPR/First Aid/AED/CPI/Naloxone. Preferred knowledge of AWARDS.

Hours:

Full-Time, 35 hours per week, variable schedule, includes one weekend day.

Pay/Salary:

40,000-44,000 Annually

Scope of Position:

Under the supervision of the Program Director, the Case Managers will be responsible for providing supportive services to formerly homeless adults diagnosed with serious mental illness, seniors, veterans, medically frail individuals, and individuals with a forensic history who are living with supportive housing. The Case Manager will hold individual meetings and co-facilitate groups while completing timely documentation in accordance with agency policy. The Case Manager will work out of an office located within the building where the client lives.

Case Manager Job Responsibilities:
  • Deliver comprehensive case management services, including housing assistance, service referrals, benefits maintenance, assessments, and coordination with external providers.
  • Work with the clinical High-Risk Coordinator to provide case management and interventions.
  • Assist in identifying strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.
  • Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing and develop natural supports.
  • Timely completion of case notes and other required documentation for every client contact as per agency policy.
  • Assist with Medication Management and Front Desk Monitoring.
  • Co-facilitate groups if requested, assist in preparing the program for audits.
  • Provide crisis intervention, as necessary, utilizing de-escalation techniques.
  • Facilitate appropriate admissions and discharges, maintaining census, documentation, and incident reporting.
  • Ability to work with consumers/residents, families, community tenants, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
  • Conduct apartment inspections on multiple floors, utilizing stairs. Provide escorts to clients throughout the community using public transportation.
  • Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Completes incident reports in accordance with Bridge policy.
  • Account for clients, performs diligent searches, and files missing person reports on consumers not accounted for in accordance with policy and procedure.
  • Attend staff meetings, supervision, and trainings as required.
  • Any other duties as may be assigned, including the potential of supporting other programs within the agency portfolio.
