Job summary

Good Shepherd Services in Brooklyn, NY seeks a Case Manager for the East NY Community Benefits Assistance Program. You will screen for benefits, assist with applications and recertifications, help with housing access, and support immigration services and referrals to public benefits agencies.

Responsibilities

  • Enroll 150 participants in the DYCD-NDA program

  • Conduct intake, assessment, and service planning with participants and enter data into required databases
  • Provide advocacy, outreach, and referrals to government agencies and other programs
  • Develop ISP plans with short- and long-term goals for NDA participants
  • Refer participants to in-house legal and financial consultants as needed
  • Coordinate with internal East NY programs to maximize community benefits
  • Maintain data tracking and write outcome reports for funders
  • Conduct community outreach to recruit new participants and promote CBAP services
  • Develop, schedule, and deliver workshops on topics like immigration, housing, financial literacy, and job readiness

Title: Case Manager, East NY Community Benefits Assistance Program

Reports To: SENIOR PROGRAM DIRECTOR, Community Benefits Assistance Program (CBAP)

Location: 2588 Avenue, Brooklyn, New York 11207

Hours: 35Hours/Week; Non-Exempt (Mon – Fri, 10 AM – 6 PM)

Position Summary: The Case Manager assists participants with a wide array of social services, including screening for appropriate government benefits, application and re-certification assistance for varied services and benefits, accessing affordable housing, applying for citizenship and other immigration services, referrals to public benefits agencies, advocacy for benefits and services, and community outreach to inform about the program’s services. The Case Manager will also work on the DYCD-NDA (Healthy Families) program, providing services similar to those of the CBAP. This is a contract position with an assignment end date of June 31, 2027.

Major Duties: Enroll 150 participants in the DYCD-NDA program

Direct Services:

  • Achieve 90 case management outcomes
  • Achieve 30 self-advocacy goals- outcomes
  • In collaboration with other Case Managers, serve approximately 2,000 clients annually.
  • Conduct respectful, accurate, and timely intake, assessment, and service planning with all assigned program participants, entering information into required databases as indicated
  • Work in an ongoing manner with participants to provide benefits screenings, direct services, and referrals to address their immediate needs as well as longer-term concerns in order to help them achieve their goals and enhance their self-sufficiency
  • Provide advocacy, outreach, and make referrals to government agencies, social service providers, elected officials, and other programs on behalf of participants
  • Enroll 20 participants quarterly in the NDA- Health Families program.
  • Develop ISP plan with short- and long-term goals for participants enrolled in the NDA program.
  • Develop self-advocacy goals with participants enrolled in the NDA program.
  • Refer participants to in-house legal and financial consultants as needed, scheduling them in shared calendars
  • Collaborate closely with other internal programs in East NY to ensure community residents benefit from the CBAP services and benefits
  • Perform related crisis intervention activities and escort participants to appointments as needed
  • Maintain appropriate use of online tracking and reporting tools and logs to program activities and outcomes
  • Compile data, complete statistical and written reports, and write-up participant vignettes and outcome stories as requested by funders
  • Perform other duties as assigned
  • Conduct outreach in the community to promote the CBAP program and recruit new participants.

Workshops:

  • Develop, schedule, and implement frequent educational and self-advocacy workshops and webinars for participants around selected topics of interest to recent immigrant families, liaising with content area experts who will deliver the workshop information.
  • Recruit and retain expert workshop presenters as well as interested workshop participants to achieve maximum utilization.
  • Schedule 1-2 monthly workshops on topics such as: Immigration, housing, financial literacy, job readiness, Worker’s rights.
  • Use required tracking and reporting databases to document services provided, outcomes, and submit proof of goal achievement.
  • Provide program updates at DYCD and internal meetings.
  • Maintain attendance and participation records and upload these into required databases to satisfy funder mandates.
  • Compile data and complete statistical and written reports as needed.
  • Perform other duties as assigned.

Qualifications:

  • Bachelor’s degree in a related field preferred
  • At least one year of related experience with government benefits and/or counseling or case management in a community-based setting required
  • Ability to work in a fast-paced office; we serve over 3,000 clients per year and have many walk-ins.
  • Must have good written and verbal communication skills
  • Must be able to work occasional evenings and Saturdays
  • Must be willing to work using a strength-based, youth and family development approach
