Job summary

Phaxis is seeking a Case Manager in the Bronx, NY to support individuals and families, especially those with SPMI, toward stability and independence.

You will assess clients, develop service plans, connect to housing, healthcare and community resources, and provide ongoing support, advocacy, and documentation. A Bachelor's degree and experience with vulnerable populations are preferred.
