Job summary

The Crisis Center of Comal County is seeking a Case Manager to provide trauma-informed, client-centered services for survivors and families. You will assess needs, develop service plans, coordinate resources, and advocate for housing and supports.

You will build relationships with community partners, document progress, and support clients from shelter to permanent housing. This role requires collaboration and compassionate service.

Qualifications

  • Bachelor’s degree in social work, human services, psychology, sociology or related field.

Responsibilities

  • Provide trauma-informed case management, advocacy, crisis intervention, safety planning, and emotional support to individuals and families in crisis.
  • conduct comprehensive assessments and develop individualized service plans focused on safety, housing stability, employment, healthcare, and long-term self-sufficiency.
  • Support clients throughout shelter stay and continue post-housing case management to maintain permanent housing and stability.
  • Coordinate shelter intake, orientation, client transitions, transportation, referrals, and connections to housing, employment, healthcare, childcare, behavioral health, legal services, and benefits.
  • Facilitate case management meetings, support groups, life-skills classes, and house meetings to promote growth and program completion.
  • Conduct home, office, community, and virtual visits to monitor progress and provide ongoing support after housing placement.
  • Build collaborative relationships with landlords, housing providers, schools, employers, healthcare organizations, and community agencies.
  • Maintain accurate documentation, electronic records, service plans, assessments, and grant reporting while protecting confidentiality.
  • Participate in reviews, trainings, and on-call rotation with shelter staff.

About the Role

As a Case Manager, you will provide comprehensive, trauma-informed case management services to individuals and families living in our emergency shelter and continue supporting them after they transition into permanent housing. You’ll assess client needs, develop individualized service plans, coordinate resources, advocate for clients, and provide ongoing encouragement as they work toward their personal goals.

This position is ideal for someone who enjoys building meaningful relationships, solving complex problems, and collaborating with both internal teams and community partners to help survivors create brighter futures.

What You’ll Do
  • Provide trauma-informed case management, advocacy, crisis intervention, safety planning, and emotional support to individuals and families experiencing domestic violence, sexual assault, homelessness, and other crises.
  • Conduct comprehensive assessments and develop individualized service plans focused on safety, housing stability, employment, education, financial wellness, healthcare, and long-term self-sufficiency.
  • Support clients throughout their shelter stay and continue providing post-housing case management to help families maintain permanent housing and achieve lasting stability.
  • Coordinate shelter intake, orientation, client transitions, transportation, referrals, and connections to community resources including housing, employment, healthcare, childcare, behavioral health, legal services, and public benefits.
  • Facilitate individual case management meetings, support groups, life-skills classes, and house meetings that encourage personal growth and successful program completion.
  • Conduct home visits, office visits, community visits, and virtual meetings to monitor client progress and provide ongoing support after housing placement.
  • Build collaborative relationships with landlords, housing providers, schools, employers, healthcare organizations, and community agencies to expand opportunities for clients.
  • Maintain accurate documentation, electronic case records, service plans, assessments, and grant-related reporting while ensuring compliance with agency policies and confidentiality standards.
  • Participate in multidisciplinary case reviews, agency trainings, and the Sexual Assault Crisis Response on-call rotation while collaborating closely with shelter staff to provide exceptional client care.

Additional Requirements
  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related social services field required; Master’s degree preferred. Equivalent experience may be considered.
  • One to two years of experience in case management, housing services, victim advocacy, homelessness services, or related human services preferred.
  • Knowledge of trauma-informed care, crisis intervention, domestic violence, sexual assault, homelessness, housing stability, and case management principles.
  • Proficiency with Microsoft Office 365 and electronic client management systems.
  • Strong organizational, communication, documentation, and time-management skills.
  • Current, valid Texas driver’s license, proof of auto liability insurance, and reliable personal transportation for local travel.
  • Ability to safely operate agency vehicles, including a van and a 14-passenger bus.
  • Ability to pass an extensive background check, including fingerprinting, drug screen, and driver’s record check.
  • First Aid and CPR certification required (may be completed after hire with agency support).
  • Ability to work a flexible schedule, including evenings, weekends, holidays, and participation in an on-call rotation.
Preferred Qualifications
  • Experience working with survivors of domestic violence, sexual assault, homelessness, or other vulnerable populations.
  • Knowledge of community housing resources, supportive services, and benefit programs.
  • Experience working with unhoused individuals and families.
  • Bilingual in English and Spanish.
  • Experience facilitating educational groups, life-skills classes, or support groups.
