Job summary

Kennedy-Donovan Center in New Bedford, MA, is seeking a Case Manager to provide direct in-home services for children and families within IFC and LIFT programs. You will assess needs, build supportive relationships, and participate in treatment planning and team meetings to ensure strong outcomes.

The role requires a bachelor’s degree in social work or related field, CPR/First Aid certification, a valid driver’s license, and proficiency in Microsoft Office.

Qualifications

  • Bachelor’s degree in Social Work or related field (Psychology, Human Services).
  • CPR/First Aid certification must be completed and maintained.
  • Valid driver’s license, acceptable driving history and reliable vehicle.
  • Proficiency in Microsoft Office.

Responsibilities

  • Assess strengths, needs, and risk factors of children, youth, and families.
  • Build trusting relationships with children, youth, foster families, and biological families.
  • Provide in-home case management, supportive services, and advocacy.
  • Participate in treatment planning, IEPs, foster care reviews, and team meetings.
  • Monitor progress toward treatment and educational goals and provide ongoing support.
  • Advocate for families and connect them with community resources.
  • Coordinate services with DCF, EEC, and external agencies.
  • Complete timely documentation and maintain regulatory compliance.
