You will collaborate with the care team to develop individualized plans, advocate for patients, manage discharge planning, and monitor progress to improve health across the care continuum at Henry Ford Health.

Qualifications

• Bachelor's degree in nursing or Master's in Social Work.
• Minimum 3 years of clinical experience.
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency in computer skills and knowledge.
• RN or LMSW with a valid, unrestricted State of Michigan license.
• Certification in Case Management (CCM) preferred.

Preferred Qualifications

• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
