Job description

Salary Range: $39,381.00 To $42,096.00 Annually

A Case Manager is responsible for providing a wide variety of services and supports to the individuals we serve in order to help them to maximize their quality of life. The position provides case management services to a group of consumers living in various community living arrangement programs or their own homes throughout the county. This includes initial intake interviews, appropriate referrals, liability information and completion of progress notes, consumer contacts as well as ongoing services such as coordination and monitoring.

A person in this position is expected to demonstrate the system of care practice model, which maximizes all resources around an individual or family. The system of care includes child and adult serving agencies and community partners that join with an individual or family to meet their goals.

Position Requirements

Education and Experience
  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
Physical Demands

While performing the responsibilities of the Case Manager, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee may be required to stand, walk, reach with arms and hands, climb or balance, stoop, kneel, crouch or crawl and, enter and exit a vehicle. The work may require constant mobility. The employee may need to work with arms above shoulder height, bend at the waist or work bent at the waist, lift or carry items or occasionally work with an uncommon level of noise. Vision abilities required by this job include close vision.

Normal office environment. The noise level in the work environment is usually quiet to moderate. Position may require travel throughout Carbon, Monroe and Pike counties in addition to periodic travel to other locations in Pennsylvania. An agency vehicle is usually available during work hours.

Position Duties
  • Assess consumer needs and develop proposed services plans/life management plans as per departmental schedules.
  • Assure implementation and coordination of the plan.
  • Monitor consumer programs as per established case management standards.
  • Maintain records, correspondence, service plans and authorizations for each consumer.
  • Make necessary referrals and coordinate services as prescribed in the consumer proposed services plan/life management plan, e.g., core team, in home therapy, camp, residential services, day program services, etc.
  • Have weekly supervisor conferences on specific cases on caseload.
  • Prepare monthly supports coordination statistics.
  • Attend staff conferences of other agencies as they relate to case management services.
  • Assure close working relationship with other human services agencies as they relate to assigned caseload.
  • Other duties as assigned by Supervisor.
  • Work will be supervised by Supervisor through regularly scheduled conferences and on an as needed basis.
Additional Duties
  • Performs related work as required.
  • Use of personal vehicle may be required.
