Housing and Support Case Manager

Key Responsibilities
  • Assist participants with identifying and addressing their housing needs and setting achievable goals.
  • Prepare, maintain, and organize all required documentation and records accurately and in a timely manner.
  • Coordinate closely with other service providers, agencies, and community resources to ensure comprehensive support for participants.
  • Develop rapport and build trusting relationships with program participants, demonstrating empathy and understanding.
  • Uphold the agency’s philosophy and principles, including choice, empowerment, cultural sensitivity, and holistic support.
  • Promote participant engagement through peer and natural supports, fostering a collaborative and respectful environment.
  • Monitor and evaluate participant progress, adjusting support plans as needed to promote success.
  • Ensure safety, accountability, and transparency in all interactions and documentation.
  • Participate in team meetings, training, and ongoing professional development to stay aligned with best practices and agency policies.
Skills and Qualifications
  • Proven experience in case management, social work, or related fields preferred.
  • Strong interpersonal and communication skills, with the ability to develop rapport and trust.
  • Knowledge of housing resources, mental health principles, and community support systems.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent organizational skills and attention to detail.
  • Cultural competence and sensitivity to diverse backgrounds and experiences.
  • Valid driver’s license and reliable transportation may be required depending on program needs.

Minimum Qualifications

Bachelor’s degree in social science OR associate degree and two (2) years human services experience preferred; OR High School Diploma/GED and four (4) years human services experience; valid driver’s license. PA State Police Background check. PA Childline. FBI Fingerprint Clearances. E.O.E
