Job summary

Avenues Recovery Center in Indianapolis, IN is hiring a Case Manager for our Detox and Residential treatment facility. You will advocate for clients, manage intakes, coordinate housing and insurance, and ensure smooth discharge planning.

Ideal candidates bring strong communication, multitasking, and problem-solving skills, with empathy and a patient-centered approach to supporting recovery.

Qualifications

• Minimum High School diploma or GED.
• 1 year experience in treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.

Responsibilities

• Advocate for client, addressing and resolving external barriers to treatment.
• Initiate case management for each client upon intake, addressing pressing needs.
• Oversee client accounts, ensuring insurance is active throughout duration of treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange housing, financial aid, legal aid, vocational training as necessary for client post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support).

Now Hiring a Case Manager for our newest Detox and Residential treatment facility, located in Indianapolis, IN.

What You'll Do

What We're Looking For
