Job summary

Socket.dev is seeking a Case Manager to identify and coordinate resources to meet the individualized needs of program participants. You will advocate for services, assess needs, and help implement each participant's service and treatment plan in collaboration with participants, staff, and community partners.

In this role you will support recovery, independence, and well-being by working with a multidisciplinary team, developing recovery plans, and connecting participants to community resources

Qualifications

  • Bachelor's degree in Human Services or related field required.
  • One year of case management experience preferred; two years in Behavioral Health, Addiction Recovery, or IDD services required.
  • Intermediate computer proficiency and valid driver's license required.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.

Responsibilities

  • Participate as a member of a multidisciplinary team, providing services from a holistic and values-based perspective.
  • Assist in the development and ongoing review of recovery plans in collaboration with program participants, clinical staff, medical professionals, and other stakeholders identified by the participant.
  • Provide community-based case management services focused on education, resource coordination, community integration, and achievement of recovery plan goals.
  • Complete all required assessments, documentation, and reports in accordance with RHD standards, funding requirements, and licensing/accreditation regulations.
  • Develop and maintain collaborative relationships with community agencies and service providers to ensure participants have access to appropriate resources and supports.
  • Advocate for participants and assist them in navigating community services and support systems.
  • Provide support at the Street 2 Feet Homeless Day Center, including administrative assistance, as needed.
  • Perform other duties as assigned.

Case Manager

Reports To: Unit Director

Position Summary

The Case Manager is responsible for identifying and coordinating the resources necessary to meet the individualized needs of program participants. This role ensures the implementation of each participant's service and treatment plan by advocating for needed services, identifying program needs and service gaps, and recommending strategies to enhance service delivery. The Case Manager works collaboratively with participants, staff, and community partners to support recovery, independence, and overall well-being.

Qualifications
Education
  • Bachelor's degree in Human Services or a related field required.
Experience
  • One (1) year of case management experience preferred.
  • Two (2) years of experience providing support to individuals in Behavioral Health, Addiction Recovery, or Intellectual and Developmental Disabilities (IDD) services required.
Skills and Certifications
  • Intermediate computer proficiency required.
  • Valid driver's license required.
