Responsibilities

• Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program.

• Helps clients obtain necessary resources and refers cases as needed.
• Documents client cases and maintains records; prepares reports for federal, state, and local programs.
• Helps organize and participate in fundraisers benefiting the program.

SUMMARY

Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Educates the community on various homebound meal outreach programs and the importance of utilizing the program
• Distributes educational materials in the community
• Visits homes to determine client needs, develops a plan to meet needs
• Identifies and monitors clients for homebound meal program
• Completes required paperwork in compliance with state and program regulations
• Creates and maintains records in SERV Tracker software
• Administers initial nutritional screening in the form of a questionnaire
• Helps client obtain necessary resources
• Refers clients with social and emotional problems to other community agencies for assistance
• Refer clients to the Director of Nutrition Services for nutrition counseling as needed
• Documents client cases, including assessments and reassessments, and maintains records on each client
• Prepares reports as required by various federal, state, and local programs
• Helps organize and participate in fundraisers benefiting the program

CERTIFICATES AND LICENSES

• Valid driver's license required
