Qualifications

• Must have a high school diploma and one to two years of related experience and/or training.
• Computer literate; able to learn SERV Tracker and Microsoft Word/Excel.

Responsibilities

• Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program.
• Educates the community on homebound meal outreach programs and importance of the program.
• Distributes educational materials in the community.
• Visits homes to determine client needs and develops a plan to meet needs.
• Identifies and monitors clients for homebound meal program.
• Completes required paperwork in compliance with state and program regulations.
• Creates and maintains records in SERV Tracker software.
• Administers initial nutritional screening via questionnaire.
• Helps clients obtain necessary resources and refers cases as needed.
• Documents client cases and maintains records; prepares reports for federal, state, and local programs.
• Helps organize and participate in fundraisers benefiting the program.
