Job Description

We are looking for a Case Manager to join our team! The case manager will be responsible for assigning, monitoring, and organizing cases for social work. They will be directly involved in the lives of their clients, and provide resources and assistance. Planning health services, working with social workers and doctors, and scheduling treatment or recovery is all in a day's work. The case manager must be highly organized, empathetic, compassionate, non-judgmental, and eager to help children and vulnerable adults.

Case Manager Duties and Responsibilities

  • Make case studies for the purpose of assessing problems and determining appropriate types and methods of treatment.
  • Investigate and provide services and referrals for clients where their physical or emotional welfare is involved
  • Work with cases involving neglect, abuse, emotional or behavioral problems, physical/mental disabilities or other health conditions affecting a client's personality, family relationships, and child or in-home care arrangements.
  • Develop intensive long or short-term treatment plans to improve or restore individual or family functioning.
  • Conduct thorough psychosocial assessments on families.
  • Develop person-centered, strength-based case plans
  • Assess the family environment to determine the potential or imminent threat to child permanency and stability.
  • Drive frequent long distances to conduct field visits in order to carry out service plans, observe, and assess client needs, and provide information and social work services
  • Maintain up-to-date records of all aspects of case management and prepares reports and correspondence.
  • Available after hours to move along cases

Case Manager Requirements and Qualifications

  • Masters' Degree in Applied Behavioral Science, preferably social work
  • Experience in case planning, case management, and fieldwork in a public or private agency as a Case Manager or closely related job title.
  • Possesses superb written and spoken communication skills
  • Excellent interpersonal skills with colleagues, community leaders, policymakers, and others
  • Excellent time management skills; organized and able to prioritize
  • Motivated to take on additional community involvement projects and solve problems
  • Comfortable in a fast-paced environment with multiple cases
  • Able to organize and manage large amounts of files, schedules, dates, and information
  • Self-directed and able to work without supervision
  • Comfortable with building personal relationships and dedicated to helping others
  • Empathetic and supportive with mentorship and leadership skills
  • Licensed or certified by state board according to regulatory
  • Bilingual - Spanish/English
