Role Description The Case Manager is a full-time, on-site role based in Houston, TX. This role involves managing a caseload of children and families, conducting assessments, and developing individualized service plans that promote safety, stability, and well‑being. The Case Manager coordinates services with foster parents, adoptive families, treatment providers, and community resources, and maintains regular contact through home visits, meetings, and check‑ins. Daily responsibilities include documenting case activity, preparing reports, and ensuring compliance with agency policies, licensing standards, and regulatory requirements. The Case Manager also participates in team meetings, case reviews, and trainings to support high‑quality care and positive outcomes for children and families.
