Responsibilities

  • Develop, implement, and regularly review individualized service and support plans.
  • Coordinate and monitor services to ensure clients receive appropriate care and resources.
  • Maintain ongoing communication with clients, families, caregivers, and service providers.
  • Track client progress and provide follow-up support as needed.
  • Accurately maintain confidential client records and documentation.
  • Prepare reports and complete required case management documentation in accordance with organizational and regulatory standards.
  • Participate in case reviews, team meetings, and service planning discussions.
  • Advocate for clients and connect them with appropriate community resources and support services.
  • Conduct in-person client visits and assessments as assigned.
  • Attend team meetings, case review meetings, and care planning sessions.
  • Coordinate services with clients, families, healthcare providers, and community organizations.
  • Complete documentation and submit reports within established timelines.
