Job summary

Paycom - ATS seeks a dedicated Case Manager to advocate for waiver participants and coordinate services in Bowling Green, KY. This role combines assessment, planning, and community resource navigation to support health and welfare.

You will conduct annual assessments, supervise monthly visits, and ensure timely documentation while collaborating with a team to uphold participant choice and access to services.

Qualifications

• BA degree in human services or related field; at least 1 year of case management experience.
• Valid driver’s license and vehicle insurance required.
• Strong organizational skills and ability to multitask.

Responsibilities

• Conduct annual assessment and person-centered service plan.
• Conduct monthly face-to-face visits with participants.
• Assist clients with accessing community resources.
• Monitor participants' progress with waiver services.
• Return calls and emails promptly and complete required documentation on deadlines.

Schedules can vary depending on participant visits. Flexible working hours.QUALIFICATIONS:BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences. One year of experience working with ID/DD individuals (ex: volunteering or having a relationship with family or friend who may have ID/DD).
