Job summary

Avenues Recovery Center in Baton Rouge, LA is hiring Case Managers to support clients through intake, treatment, and discharge planning. You will advocate for clients, address barriers to care, and coordinate housing, financial aid, and legal resources to ensure continuity of treatment.

Ideal candidates have a High School diploma (GED) and at least 1 year of experience in the treatment field, with strong communication and organizational skills.

Qualifications

• High School diploma or GED preferred.
• Minimum 1 year of experience in treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Warm, empathetic, respectable personality.

Responsibilities

• Advocate for client and resolve barriers to treatment.
• Initiate case management at intake, addressing pressing needs.
• Oversee client accounts and ensure insurance stays active.
• Provide ongoing case management throughout treatment.
• Arrange housing, financial aid, legal aid, vocational training etc. post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support).

Now hiring Case Managers for our facility in Baton Rouge, LA!

What You’re Doing:

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What We’re Looking For:
