Job summary

CVS Health is seeking a Case Management Coordinator for a work-from-home role with travel to the West Valley, AZ. You will coordinate, document, and communicate care management activities, applying evidence-based practices to support members.

Qualifications include 2+ years in case management for SMI or elderly, and a Bachelor’s degree or Arizona RN license. Strong MS Office skills and bilingual English/Spanish are a plus.

Qualifications

• 2+ years in case management for individuals with serious mental illness or elderly care.
• Must reside in West Valley, Arizona, with up to 50% travel within West Valley.
• Bachelor’s degree in Social Work, Psychology, Special Education, or Counseling preferred.

Responsibilities

• Coordinate and document care plans, and monitor progress using care management tools.
• Evaluate member needs/eligibility and recommend appropriate actions.
• Engage providers and families to support medical and lifestyle goals.
• Ensure compliance with regulatory and accreditation guidelines.
