This job description should not be interpreted as all-inclusive. It is intended to identify the essential functions and requirements of this position. The incumbent may be requested to perform job-related responsibilities and tasks other than those stated in this job description.
Case management is an essential function of this position and requires professional knowledge and experience in assessing client needs, developing individualized service plans, establishing measurable goals, monitoring and documenting client progress, coordinating services, maintaining accurate case records and HMIS documentation, facilitating supportive programming, and developing appropriate discharge and transition plans. The position requires the ability to evaluate complex client circumstances, identify barriers to stability and self-sufficiency, coordinate multiple internal and external resources, make appropriate referrals, monitor outcomes, and modify service interventions as necessary. These responsibilities require demonstrated knowledge and application of social service case management principles and the ability to work effectively with individuals experiencing complex social, behavioral, housing, and economic challenges.
Given the population served, experience providing case management services to individuals impacted by domestic violence is a directly relevant and valuable qualification for this position. Domestic violence cases may involve safety risks, trauma, coercive control, housing instability, financial dependence, mental health concerns, and other circumstances requiring specialized assessment and service coordination. The individual in this role must be able to apply trauma-informed and safety-focused practices, maintain appropriate confidentiality, develop individualized goals without increasing client risk, and coordinate specialized community resources. Requiring or strongly preferring demonstrated domestic violence case management experience is therefore reasonably related to the essential functions of the position and supports the organization’s responsibility to provide competent, appropriate, and effective services to a vulnerable client population.
