Qualifications

• Bachelor's degree in social welfare or related field with 12 credits in social sciences.
• Knowledge of current social case management principles, techniques, and methods.
• Ability to conduct individual and family interviews to identify problems.
• Ability to establish and maintain effective working relationships with clients, families, staff, and outside agencies.

Responsibilities

• Provide case management services.
• Maintain face-to-face and telephone contact with assigned consumers and collaterals per regulations.
• Spend 75% of working hours in contact with assigned persons and collaterals, including travel.
• Spend at least 50% of working hours outside of the office.
• Attend training opportunities designated by OMH for Intensive Case Managers.
