Interim HealthCare Inc. in Fort Wayne, Indiana is seeking a Case Management Registered Nurse (RN) to coordinate patient care plans across providers and services.

You will assess clinical, psychosocial, and functional needs to identify appropriate interventions and advocate for quality care and safe transitions. With a minimum of 2+ years as an RN Case Manager, an active Indiana RN license and CPR certification are required.

Qualifications

• Active RN license in Indiana required.
• Minimum 2+ years as an RN Case Manager.
• Current CPR certification from the American Heart Association.
• Case Management certification preferred.
• Strong assessment, critical thinking and collaboration skills.

Responsibilities

• Coordinate and manage patient care plans across providers and services.

As a Clinic Nurse (RN), you will:

• Coordinate and manage patient care plans across healthcare providers and services.
• Assess patients' clinical, psychosocial, and functional needs to identify appropriate interventions.
• Collaborate with physicians, nurses, social workers, insurance companies, and community resources.
• Monitor patient progress, outcomes, and barriers to care while adjusting plans as needed.
• Advocate for patients by promoting quality care, safe transitions, appropriate resource utilization, and continuity of care.

A few must-haves for Clinic Nurses:

• Graduate of an accredited nursing program with an active RN license in Indiana
• Minimum of 2+ years of experience as an RN Case Manager
• Current CPR certification from the American Heart Association
• Prefer certification in Case Management
• Strong assessment skills, critical thinking, and the ability to work independently
• Compassionate communicator who values collaboration with students, staff, and families
