Job summary

The Valley Hospital in Paramus, NJ is seeking a Clinical Care Coordinator, Case Manager to coordinate patient progression, discharge planning, and transitions of care across the continuum. The role collaborates with patients, families, physicians, payers, HORC, and the interdisciplinary team to promote appropriate resource utilization, timely placement, and compliant care delivery.

Requires a BSN, RN licensed in New Jersey, and at least five years of clinical nursing experience; ANCC Case

Qualifications

• Bachelor's degree in nursing (BSN) required.
• Registered Nurse license in New Jersey.
• ANCC Case Management Certification preferred.

Responsibilities

• Coordinate patient progression and transitions of care across the continuum.
• Oversee discharge planning and admission status review.
